Independent Contractor Determination Form

The recently released Independent Contractor Determination Form (ICDF) is used to determine whether an individual should be set up as an independent contractor or a UA employee.  The new fillable form is designed to be user friendly for the department seeking services and for the contractor by asking a series of Yes/No questions with selectable radio buttons rather than lines of text. This form replaces all previous versions of the form including the Employee vs. Contractor Checklist.
