Overview

Dynamic Forms is a tool used to develop new online electronic forms, specifically those that need workflow logic. Dynamic Forms requires electronic signatures, that become legally binding. Dynamic Forms is a cloud-based form system that caters to educational institutions. It authenticates from the University of Alaska's Single Sign-on (SSO). Non-SSO users can set up an account with Dynamic Forms.

How do I use this technology?

Before you can log in, you must be an active University of Alaska (UA) student or employee, meaning:

  • Students who have registered for at least 1 credit through UAOnline within the past year, or
  • Employees with an active job status

Is there any additional information I should know about?

University of Alaska Fairbanks (UAF) Nanook Technology Services (NTS) & UA Office of Information Technology (OIT) are responsible for provisioning Dynamic Forms site owners, otherwise referred to as the organization owners. Any requests to make changes to an existing Dynamic Forms form go through the form owners, not UAF NTS & UA OIT.

Need additional help or have issues

If your UA credentials work everywhere else (e.g., UAOnline, Google Workspace, ME.UAA, etc.) besides Dynamic Forms, please contact UAF NTS & UA OIT, or submit a ticket here.

For form owners and managers having issues with specific features, or something is not working as expected, please contact UAF NTS & UA OIT, or submit a ticket here.

For end-users experiencing issues accessing a form provided by a department, please contact that specific department. For instance, if the form was distributed by UA Human Resources (HR), you should reach out to UA HR for assistance.

For other technical issues, requests may be submitted anytime using the Support for Surveys and Online Forms form. Requests generate a Ticket which will be worked in order received and urgency by UAA Employees with the knowledge and permissions to assist with the request.
