This service is used to order a new VoIP telephone, request updates to a telephone, or remove/decommission a telephone. This form can also facilitate requests for multiple phones.
Eligibility
Required Information
Before submitting a ticket, please have the following information ready:
- Type of phone requested (2-Line or 6-Line)
- Additional "sidecar" line expansion module
- Building and room where phone will be installed, moved to, or picked up from
- Wall port number where phone needs to be plugged into or is currently plugged into
- Org and Fund numbers with point of contact for billing inquiries
